What is bipolar disorder?

Bipolar disorder is a mood disorder marked by episodes of depression and episodes of abnormally elevated or irritable mood, called mania or hypomania. Between episodes, many people feel like themselves. According to the National Institute of Mental Health, about 2.8% of U.S. adults experience bipolar disorder in a given year.

Types of bipolar disorder

  • Bipolar I: at least one manic episode lasting a week or more (or severe enough to need hospital care). Depressive episodes are common too.
  • Bipolar II: episodes of hypomania, a milder form of mania, along with major depressive episodes. Depression is often the most disabling part.
  • Cyclothymic disorder: ongoing mood swings between milder highs and lows lasting two years or more.

Signs to watch for

During mania or hypomania: much less need for sleep, racing thoughts, rapid speech, unusual confidence, risky spending or decisions, and taking on many projects at once.

During depression: low mood, loss of interest, fatigue, changes in sleep and appetite, trouble concentrating, and hopelessness.

Why an accurate diagnosis matters

Bipolar disorder is often first diagnosed as depression, especially bipolar II, because people tend to seek help when they feel low, not when they feel energized. This matters because some treatments for depression can trigger mania in people with bipolar disorder when used alone.   • Routine and sleep. Regular sleep and daily rhythms are among the most powerful protective factors against mood episodes.
